The Medal of the Immaculate Conception Virgin Mary Circular Pendant is built in three levels, and the depth is what separates it from a stamped medal.
A mirror-polished rim runs the full circumference. Inside it sits a pointed Gothic star frame, also polished, its points reaching out toward the rim. And set down inside that is a recessed oval panel in fine satin, where the whole devotional image lives.
The Virgin stands full-length at the centre in high relief, veiled, her hands opened downward and out with rays streaming from her fingers in long carved lines. Beneath her feet is the globe, and around it the serpent, worked in coils at the base of the panel. Her halo is raised behind the head and the robe falls in deep folds all the way to the hem.
Around the oval runs the inscription — MARIA IMMAC · ORA PRO NOBIS, “Mary Immaculate, pray for us” — raised and polished against the satin ground, with a ring of small carved star-flowers following the frame line the whole way round.
The spandrels outside the oval carry fleur-de-lis scrollwork and rosettes, and interlaced Marian monogram cartouches at the upper and lower corners.
The design comes from the apparitions to Catherine Labouré at Rue du Bac in Paris in 1830. The first medals were struck in 1832, and it became so widely associated with answered prayer that people simply began calling it the Miraculous Medal.
